The first step in the certification process is to check the eligibility of each applicant. Please download the two forms below, complete them and return them to:

Please complete the forms in full and attach copies of certificates and/or proof of any relevant qualifications. Please also attach a copy of your personal CPD log. Also select your preferred method of payment for the certification process.
